Contact information about You to be included in the Alumni Book

    Privacy:  Info added here will be added to a password protected PDF file view-able only by you and your classmates.  (see more on the View Alumni Book page).  Don't forget to click the submit button after filling out the contact info and click the 2nd submit button after filling out the survey.

    Password:  Once you submit your information and we confirm that you are a classmate, we will add your entry to the Alumni Book, and email you a password. Editing: If you want to edit your information after submitting the form, please resubmit a new form.

    If you don't want your information in the Alumni book but just want to update the class records please say so in  "Tell us about You."

    This survey is for fun and the results will be revealed at the dinner.
